Intake Process
Our patient intake process ensures a smooth transition into treatment. The process officially begins when we receive information to schedule an appointment and the patient shows up for the appointment. The steps that follow are the same for any new patient:
Intake Process
Prior to your first session, you will be asked to complete a client intake packet. This process is crucial in forming a therapeutic relationship with your therapist and gathering information about yourself. Please do not hesitate to discuss any question that you may have regarding this form with your therapist during your first visit.
The first session generally involves completing a Comprehensive Clinical Assessment (CCA) and answering questions about different areas of your life—including reason(s) why you came to therapy, your social, medical, mental health, substance use history, education, employment, and more.
Next, your therapist will begin working with you on developing an appropriate and detailed plan with treatment goals, measurable objectives, and a timeline for your treatment progress. This plan will allow your therapist to track your progress and guide you towards reaching your goals.

Counseling Intake Process
Intake
Paperwork is completed and preliminary information is gathered during this stage.
Exploration
Patients provide details about the rationale to seek treatment and set up goals and expectations.
Intervention
Patients learn new techniques and how to apply them to health and achieve personal growth.
Empowering
Patients learn how to use the skills that they have learned to develop independence and self-confidence.
Maintenance
Patients design prevention plans and test the durability of recently learned skills.
Online Intake Process
Online Scheduling
We offer the option to schedule appointments online for convenience and to give you the intake process.
E-Forms
With online intake forms, you can instantly upload information and access your electronic health record. This reduces errors and saves time.
Digital Communication
We can implement text and email communication between you and us as well as send alerts via email, phone, or text.
Patient Access Portal
You can use a portal to access information, make payments, schedule appointments, and communicate with the office.
Admission Criteria
We use evidence-based clinical standards of care to determine clinical necessity and appropriateness, a week to evaluate, diagnose, and treat medical and psychiatric conditions.
Previously completed an inpatient level care and needs to continue receiving services as an outpatient.
Existing psychiatric symptoms are clinically significant or profound that create impairment in day-to-day social, vocational, and/or educational functioning.
Requires intensive treatment, but not 24-hour observation and inpatient care.
Experiencing unmanageable mental health symptoms that are interfering with daily life.
Use of illegal substances, changes in mental state, and decrease in social, mental, and occupational functioning.
