Wilmington Mental Health uses clinical assessments to know the type of treatment that must be used to effectively treat you and meet your needs. They collect information and draw conclusions about the symptoms you are presenting from these assessments, including your demographic information, sociocultural background, personal characteristics, cognitive and functional functioning, cultural factors, history of mental health, substance abuse treatment, as well as your skills, struggles, and treatment expectations.
Your clinical assessment is an ongoing process that helps your therapist determine if treatment is needed, the extent to which your psychological condition is affecting you, whether the treatment method that has been selected is working, and if your symptoms are likely to return after treatment has ended.

Comprehensive Clinical Assessment (CCA)
The CCA is an intensive clinical and functional face-to-face evaluation of your presenting mental health, developmental disability, and substance use disorder from which a written report is created to provide the clinical basis of your treatment and service plan. A CCA is needed when there is a known or suspected mental health or substance use disorder, or initial screening or triage information indicates a need for additional mental health or substance use disorder.
The Clinical Interview
One of our licensed professionals will complete a CCA with you upon admission to determine medical necessity and the level of care needed. Relevant diagnostic information will be included in a final report.
Your CCA will include the following elements:
- The presenting problems
- Chronological general health and behavioral health history
- Current medications
- Biopsychosocial and developmental dimensions
- Case formulation
- Diagnosis from the DSM-5
- Recommendations for additional assessments, services, support or treatment
Mental Status Exam
A Mental status examination is used to organize the information collected during the interview and to systematically evaluate you through a series of observations and questions assessing your appearance and behavior, thought processes and content, mood and affect, intellectual functioning, and awareness of surroundings.

Suicide Risk Assessment

Suicide Ideation means thinking about or planning suicide, thoughts can range from a quick consideration to a detailed plan.
Considerations
The assessment is culturally sensitive, preventative, explorative, fluid, collaborative, strengths based, and engaging. Suicidal thoughts are common and this assessment allows every individual to get the best care and guidance in the least restrictive setting. Treatment will focus on reducing the immediate suicide risk and exploring the underlying mental health and/or substance use disorder.
Assessment Steps
- Risk Factors: We will take a look at traumas, triggering events, medical health, physical health, purpose, mood, etc., to identify risk factors.
- Identify Protective Factors: What prevents an individual from having suicidal thoughts, ideations, and behaviors.
- Conduct a Suicide Inquiry: Learn more about ideation, any plans, behaviors, intents, ambivalence, and any other notes.
- Determine Risk Level: Whether an individual is a high, moderate, or low risk.
- Determine Intervention: This is based on the scope and role, available resources, referrals, and polices/procedures.

- Recognize Your Personal Warning Signs: What thoughts, images, moods, situations, and behaviors indicate to you that a crisis may be developing? Write these down in your own words.
- Use Your Own Coping Strategies: List things you can do on your own to help you not act on urges or to harm yourself.
- Contact Family Members or Friends: They may be able to help resolve a crisis. Make a list of people who are supportive and who you feel you can talk to when under stress.
- Contact Mental Health Professionals or Agencies: Make a list of names, numbers, and/or locations of clinicians, local emergency rooms, and crisis hotlines.
- Ensure Your Environment is Safe: Have you thought of ways in which you might harm yourself? Work with a counselor to develop a plan to limit your access to these means.
