These visits may include: A full-body physical examination, checking your vitals and BMI, reviewing your medical history, discussing your current lifestyle and health choices, screening, administration of shots and immunizations, and establishing a health plan.

INTAKE QUESTIONS
Primary Care Team

We want to develop a long-lasting relationship with our patients. Therefore, is important that you feel comfortable with your treatment team. If you are not comfortable being 100% honest with your healthcare provider, think about switching to someone else.
Your wellness exam is an opportunity for you to learn more about your own body. Feel free to ask questions during this time. If you need any tests, ask our team if we can do it on site. If we do not, we will refer you to a local specialist.
INTERDISCIPLINARY WORK
Prevention vs. Treatment

Our goal is to offer preventive healthcare to keep you healthy in general. This approach contrasts the traditional treatment focus that addresses diseases or problems that could have been prevented. Disease prevention is cost effective and reduces the risk of suffering, death, or high health care cost, especially when a particular disease has reached the chronic stage.
There are, nevertheless, barriers that prevent people from seeking help, including the cost of healthcare, lack of time or knowledge, transportation, and stigma, among others. Our team can implement evidence-based intervention, provide immediate access to healthcare and lab tests, screen for coexisting problems, integrate physical and behavioral health, and bridge the gap between therapy and primary care. This integrated approach to care can potentially restore your psychological health and speed healing.
